Acute renal failure (also called acute kidney injury) means that your kidneys have suddenly stopped working. Kidneys remove waste products and help balance water and salt and other minerals (electrolytes) in your blood. When your kidneys stop working, waste products, fluids, and electrolytes build up in your body. This can be dangerous and needs prompt attention from a physician.
Chronic kidney disease (CKD) is a condition characterized by a gradual loss of kidney function over time. It includes conditions that damage your kidneys and decrease their ability to keep you healthy by doing the jobs listed. You may develop complications like high blood pressure, anemia (low blood count), weak bones, poor nutritional health and nerve damage. Also, kidney disease increases your risk of having heart and blood vessel disease. Chronic kidney disease may be caused by diabetes, high blood pressure and other disorders. 

Early detection and treatment is important to identify and treat potentially reversible kidney problems, or to slow or even halt progression of chronic kidney disease. When kidney disease progresses, it may eventually lead to kidney failure, which requires dialysis or a kidney transplant to maintain life.
Dialysis is the method of cleaning your blood and removing fluid and toxins after kidneys fail.  With end-stage kidney disease, kidney functions can be replaced only by dialysis or by kidney transplantation. The planning for dialysis and transplantation usually starts in stage 4 of chronic kidney disease.
